Travel bookings pass through more hands than almost anything else online. A single reservation can touch the site you booked on, a global distribution system, the airline or host's own platform and a payment processor, and any one of them failing looks the same from the outside.
The practical consequence is that an outage here rarely destroys a booking, it just hides it. The reservation exists in a system you cannot reach, which is why the confirmation email matters more during an incident than the app does.

Almost never. Once a reservation is confirmed it lives in the airline or host's system, not only in the site you booked through. The confirmation email is the record, and it stays valid while the website is unreachable.
Wait and check your email and card statement first. Bookings that appear to fail during an incident frequently completed, and a duplicate reservation is much harder to cancel than to avoid.
